The Fracture.
Kolya came to the dentist, scared.
“Don’t be scared,” calmed the dentist, “First, I will tell you everything.”
Leonid Konstantinovich always calmed his patients before healing them. He felt that the patient was sacred not of the pain, but of the unknowingness that comes with it. This is why Leonid Konstantinovich always told his patients about the process beforehand.
“First we will numb with the shot, and next we will make a hole with the drill, pick out the nerve with a wire, fill it with cement, dry it, and smooth it down.” said Leonid Konstantinovich, “If the tooth starts hurting even more on the next day, then we will root it out, in other words, we'll pull it out with tweezers. Now you understand everything and aren’t scared.”
Leonid Konstantinovich put on a gauze mask and picked up the syringe.
And it was true: after learning about the treatment coming up, Kolya wasn’t scared. He felt like a policeman from Susquehanna, Pennsylvania. He felt like the critical moment had come. With a sudden movement, Kolya grabbed the syringe from Leonid’s hands, placed it into his pocket with the words, “I’ll keep this toy with me for now,” and slowly left the office.

You might think about the other meaning of the work "Перелом" for the title.
